Parent & Teen Waivers
All attendees and their legal guardians must complete the required CISAITSS safety and behavioral waivers prior to event admission.
Submission Instructions
Completing the waiver process is mandatory. We use a secure, centralized Jotform to collect parent consent, medical disclosures, and the teen behavioral agreement. Follow the steps below.

Parent/Guardian Section
The parent or legal guardian completes the first section, including emergency contacts and consent acknowledgment.
Teen Acknowledgment
The attending teen completes the behavioral expectations section and provides their electronic signature.
